I find that setting goals keeps me more motivated. I don't mean weight loss goals, because everyone's different and exercise and diet will affect you differently. That gets discouraging pretty quickly, esp since you are certain to plateau at some point. So pick something attending a cardio class for a whole semester, being able to run one mile, or two without stopping. Once you have your first goal mastered, pick a second, like running or biking a race or being able to bench press X amount of weight. By evolving your goals you can both measure your progress and keep challenging yourself, which keeps you from getting bored. Try different activities until you find the one that works for you and your schedule. Even just walking for 30 min in the evening will help, and you can take your baby with you. (Which has the added benefit of teaching good exercise habits to your child).

As for the diet, I have one thing to say: EAT MORE VEGGIES. I have trouble with this too, but its important. Right now it seems like most of your vitamins are coming from orange juice. Try substituting an actual orange for the juice... you'll get the added benefit of fiber, etc.

I am assuming you are eating Mc Donalds for lunch because you don't have time/inclination to pack a lunch. Attempt to find time to pack your lunch because you will be able to make healthier choices (more veggies). If this isn't possible, try some of the healthier fast food options, like swapping out the french fries for a salad (at least a couple days a week).
